Tomo, Inner fire mediatation? Mediation in General? Please help?

Also when I mediate, (Buddhist mediation) they say to clear your mind or all those and focus on your breathing. Someone said in tomo you are envisioning something. Which is true? what do I do? thank you very much for your help
